We are currently seeking dedicated individuals for the role of District Retention and Linkage Officers.

Location: Kigamboni Municipal Councils

Reports to: City/District AIDS Control Coordinators

Duties and Responsibilities:

  1. Implement effective tracking and retention services to ensure 95% adherence to ART and clinic schedules for on ART clients.
  2. Provide technical assistance to clinical trackers to reduce quarterly net loss by 70% and document best practices.
  3. Foster collaboration with community partners to facilitate 95% linkage to care for newly identified clients in both community and facility settings.
  4. Offer technical support to healthcare providers on ART adherence counseling, with a focus on PLHIV with High VL.
  5. Support peer navigators/CBHS Plus to link all PLHIV diagnosed in the community to care and ensure proper documentation.
  6. Mentor and provide TA to health providers to ensure successful referral and re-engagement into care.
  7. Provide TA to healthcare providers on the management of STIs/RTIs, GBV, and Cervical cancer, including guideline availability, protocols, and reporting tools.
  8. Support the provision of PHDP services designed to help PLHIV live positively.
  9. Ensure the facility has and uses National follow-up tools and tracking systems.
  10. Improve the retention rate of clients on care.
  11. Coordinate and facilitate monthly meetings for sharing challenges and improvement strategies in tracking activities.
  12. Support sites in understanding targets, aligning strategies, and meeting performance targets.
  13. Work with trackers and data officers to ensure timely updates of tracking outcomes.
  14. Collaborate with CHMT to ensure quality monthly reports for tracking and retention in care.
  15. Perform any other duties as assigned by the supervisor.

Requirements:

  • At least a Degree in Medicine or Advanced Diploma in Medicine, Clinical, or Nursing Officer with three years of relevant work experience.
  • Two or more years of experience in HIV/AIDS care and treatment programs.
  • Minimum of 2-3 years’ experience in tracking and follow-up activities.
  • Ability to write reports and prepare planning.
  • Experience working with CHMT will be an added advantage.
  • Ability to maintain confidentiality.
  • Excellent communication skills in both English and Kiswahili.
